About The Position
Eddy Visiting Nurse & Rehab Association is seeking a part-time Specialty RN to care for patients in their homes in a designated area of Saratoga County. The role uses an interdisciplinary approach, with team discussion of complex cases, and focuses on one patient at a time. The RN teaches patients and caregivers about diagnoses, medications, treatments, and chronic illness self-management. The position offers opportunities to develop skills in wound vacs, advanced wound care, IV care, and disease management. After training, the RN can start and end the workday from home. The association provides a comprehensive orientation to home health care, clinical nurse specialist access, computer training, help obtaining insurance authorizations, and phone support from RN supervisors. Candidates may shadow an experienced RN Case Manager.
Requirements
- NYS RN license and registration.
- At least 1 year of RN acute care experience.
- Valid New York State RN and DMV licensure.
- Strong communication skills for working with interdisciplinary team members and family members.
- Strong critical-thinking skills.
Responsibilities
- Teach patients and caregivers about diagnoses, medications, treatments, and self-management of chronic illnesses.
- Provide home health care using an interdisciplinary approach and discuss complex cases as a team.
- Care for one patient at a time.
