About The Position

The Registered Nurse Case Manager delivers care to patients and families with needs ranging from basic to complex, using the nursing process. The role creates a therapeutic and caring environment for patients and families and practices consistently with the Elliot Hospital Interprofessional Practice model, the ANA Social Policy Statement, Scope and Standards of Practice, the Nursing Code of Ethics, and relevant specialty standards. The position includes weekend and holiday rotation and backup on-call responsibilities. The VNA provides 24/7 in-home support and services to patients in Manchester and surrounding communities, helping them recover from surgery or cope with life-limiting illnesses or physical disabilities.

Responsibilities

Deliver care to patients and families with needs ranging from basic to complex using the nursing process.
Create a therapeutic and caring environment for patients and families.
Embrace and promote change to support optimal patient outcomes and an effective professional practice environment.
Involve patients and families in goal setting and planning, communicate the plan of care to them for input, and share it with colleagues to ensure seamless care across shifts and settings.
Remain accountable for continued professional growth and development and for contributing to smooth operations.
Recognize and manage rapidly changing situations.
Use current evidence and relevant national and specialty standards to guide practice and identify areas for improvement.
Demonstrate competent clinical practice using clinical decision-making and critical-thinking skills.
