Social Worker - Acute Care
About The Position
Emplify Health is seeking a part-time Social Worker to join its acute care Social Services team at the La Crosse main campus. The role provides services to acute care patients and supports individualized treatment plans through assessment, coordination, navigation, and planning. Social workers also serve as consultants and leaders in comprehensive care across the system while supporting organizational strategies and goals. The position is scheduled for 40 hours bi-weekly during core business hours, with a two-week schedule: Wednesday, Thursday, and Friday in Week 1, and Monday and Tuesday in Week 2. There are no weekends, with one holiday per year on a rotation. Starting wage is $25.95 per hour, with additional compensation for comparable experience. Requirements
- Bachelor’s degree in Social Work or a related field.
- Wisconsin Social Worker license.
Nice To Haves
- One year of work experience in social services in a hospital, medical clinic, county social service agency, or nursing home.
Responsibilities
- Provide acute care patients with discharge planning, care coordination, care navigation, resource connections, supportive listening, advance care planning completion, and long-term care planning.
- Assess complex biopsychosocial needs and provide interventions that support individualized treatment plans.
- Act as consultants and leaders in comprehensive care throughout the system.
- Support organizational strategies and goals.
