About The Position

The Social Worker will join the Social Services team and provide services to acute care patients at Emplify Health’s La Crosse main campus. This is a part-time position scheduled for 40 hours bi-weekly during core business hours. The schedule is Wednesday, Thursday, and Friday in week one, and Monday and Tuesday in week two. The position does not require weekends and participates in a rotation for one holiday per year. The role supports patients through discharge planning, care coordination and navigation, resource connections, supportive listening, advance care planning completion, and long-term care planning. The Social Worker assesses complex biopsychosocial needs and provides interventions that support individualized treatment plans. They also serve as consultants and leaders in comprehensive care throughout the system while supporting organizational strategies and goals. Starting wage is $25.95 per hour, with additional compensation for comparable experience, plus a compensation and benefits package.

Requirements

Bachelor’s degree in Social Work or a related field.
Wisconsin Social Worker license.

Nice To Haves

One year of work experience in social services in a hospital, medical clinic, county social service agency, or nursing home.
