About The Position

Agape Care Group is seeking a hospice medical social worker to provide psychosocial evaluations and ongoing counseling to patients and families during the end-of-life journey. Working in accordance with the plan of care, the social worker provides emotional support to patients and families. Requirements

A heart to serve patients and families and a passion for providing the best possible care.
Master of Social Work degree from an accredited school of social work approved by the Council of Social Work Education.
Current state license as a social worker.
At least 2 years of clinical work experience; healthcare or hospice experience is preferred.
Reliable transportation.
Ability to sit, stand, bend, move intermittently, lift at least 25 pounds, and effectively bear the weight of an average adult.
