The hospice social worker provides emotional and logistical support to patients and their loved ones during end-of-life care. The role helps patients put their affairs in order in accordance with their wishes, including financial and funeral arrangements, and supports loved ones as they navigate grief. The social worker conducts psychosocial assessments of patients and families and collaborates with clinical teammates to develop comprehensive plans of care. Responsibilities include individual and family counseling, psychotherapy when appropriately credentialed, proactive bereavement care after a patient's passing, referrals to community resources, and timely, accurate documentation. Most work takes place where hospice patients live, including private residences and assisted living or skilled nursing facilities. About once a week, the social worker comes to the office to meet with the interdisciplinary team.
