About The Position

Hospice Social Workers support people in the final stage of life and their families by providing emotional and practical assistance aimed at enhancing their well-being and quality of life. They work closely with healthcare teams to organize treatments, services, and available resources. This full-time position serves St. Louis, Missouri, and surrounding areas, with a Monday through Friday schedule from 8 a.m. to 5 p.m. Counseling is provided in patients’ homes.

Requirements

• Master’s degree in Social Work or Licensed Clinical Social Worker (LCSW).
• Ability to work in a professional manner.
• Ability to work well independently and in a group setting.
• Professional appearance.
• Excellent attendance and punctuality.
• Problem-solving skills and ability to multitask.
• Excellent communication skills.
• Positive, professional attitude.

Nice To Haves

• Hospice experience preferred.

Responsibilities

• Provide individual and family counseling in the home setting.
• Assist with establishing advance directives and durable power of attorney.
• Facilitate use of community and home care resources.
• Attend and participate in IDG.
• Make referrals to long-term care facilities as needed.
• Develop and facilitate support groups.
• Meet with other social workers and the hospice team.
• Maintain professional relationships with staff and referral sources.
