About The Position

21st Century Home Health Services is hiring a Clinical Social Worker (MSW or LCSW) to serve patients in the San Mateo/Palo Alto territory. The role assists patients and their families or caregivers with education and support as they cope with a recent diagnosis. Much of the patient population is geriatric, so the work requires knowledge of local community resources and short- and long-term care planning. The position requires field work and visits to patients in their homes. A primary territory will be assigned, though temporary coverage outside it may occasionally be needed to support vacancies, leaves, patient census, or other care needs. Under the general direction of the Case Manager, the Social Worker assesses social and emotional factors related to the patient's illness and medical and nursing requirements in the home. The Social Worker develops and implements a plan for home social work services, including educating patients, family members, and caregivers, and coordinates with home health staff and community resources to integrate care efficiently. This is a per diem role. The employer offers flexible schedules and reimbursements for mileage, parking, CPR, and continuing education units. The listed annual pay range is $80,000–$90,000.

Requirements

Master's degree from a program accredited by the Council on Social Work Education.
Basic Life Support (BLS) certification.
Driver license.
Automobile insurance.
Knowledge of local community resources.
Knowledge of individual development and human behavior as related to the effects of illness or injury, and of the influence of culture on health care.
Effective written and verbal communication skills.
Ability to recognize the needs and concerns of diverse groups of people.
Ability to maintain constructive working relationships with patients, families, interdisciplinary team members, clerical and management staff, physicians, and community resources.
Well-developed social work assessment and clinical skills, and time-management skills.
Ability to handle sensitive issues and conflict respectfully; plan, organize, and prioritize work; think critically; solve problems or identify when to consult a supervisor; and manage time demands and occasional conflicting priorities.
Maintain a positive, professional image and pursue self-directed learning to meet professional development or license requirements.
Maintain all required licensure and certifications.
Meet core competencies for the position and area of specialty at orientation and annually thereafter.

Nice To Haves

LCSW license is highly preferred; supervision is provided to license-eligible social workers.
Knowledge of state and federal home-care regulatory guidelines is preferred.
One year of experience as an MSW in a health care setting is strongly preferred.
Home health experience is preferred.

Responsibilities
