About The Position

Hospice Social Workers provide emotional and practical support to people in the final stage of life and to their families and close ones. They collaborate with healthcare teams to organize treatments, services, and resources, with the goal of enhancing patients’ well-being and quality of life while confronting end-of-life challenges. This position serves St. Louis, Missouri, and surrounding areas, and provides care in home settings.

The role includes counseling patients and families, connecting them with community and home care resources, helping establish advance directives and durable powers of attorney, and coordinating with hospice colleagues and referral sources. The schedule is full-time, Monday through Friday, 8 a.m. to 5 p.m.

Requirements

• Master’s degree in Social Work or Licensed Clinical Social Worker (LCSW).
• Ability to work in a professional manner.
• Ability to work well independently and in a group setting.

Nice To Haves

• Hospice experience preferred.

Responsibilities

• Provide individual and family counseling in the home setting.
• Assist with establishing advance directives and durable powers of attorney.
• Facilitate the use of community and home care resources.
• Attend and participate in IDG.
• Make referrals to long-term care facilities as needed.
• Develop and facilitate support groups.
• Meet with other Social Workers and the Hospice team.
• Maintain professional relationships with staff and referral sources.
