About The Position

21st Century Home Health Services is hiring a Clinical Social Worker to provide home health social work services in Northern California. The position works under the general direction of the Case Manager and requires field work, including visits to patients in their homes. The Social Worker assesses social and emotional factors related to patients’ illnesses and their medical and nursing needs in the home. The role develops and implements plans for home social work services, educates patients, family members, and caregivers, and coordinates with home health staff and community resources to support integrated care.

Requirements

Master’s degree from a program accredited by the Council on Social Work Education.
Basic Life Support (BLS) certification.
Driver license.
Automobile insurance.
Knowledge of local community resources.
Knowledge of individual development and human behavior in relation to the effects of illness or injury, and of cultural influences on health care.
Effective written and verbal communication skills.
Ability to recognize the needs and concerns of diverse groups of people.
Ability to maintain constructive working relationships with patients, families, interdisciplinary team members, clerical and management staff, physicians, and community resources.
Well-developed social work assessment and clinical skills, and time-management skills.
Ability to handle sensitive issues and conflict, direct and reinforce staff efforts, plan and organize work, prioritize, think critically, solve problems, and identify when to consult a supervisor.
Ability to manage time demands and occasional conflicting priorities in a challenging work environment.
Responsibility for maintaining all required licensure and certifications.

Nice To Haves

LCSW license; supervision is provided to license-eligible Social Workers.
Knowledge of state and federal homecare regulatory guidelines.
One year of experience as an MSW in a health care setting.
Home Health experience.

Responsibilities
