About The Position

Brookestone Home Health & Hospice, a proud member of the Vetter Senior Living family, is seeking a compassionate and dedicated Social Worker (MSW) to join our Home Health and Hospice team. In this rewarding role, you will provide emotional support, advocacy, education, and resources to patients and families as they navigate health challenges and life transitions. Working alongside nurses, therapists, physicians, and other healthcare professionals, you'll help ensure patients receive the support they need to maintain the highest possible quality of life. Requirements

• Master's Degree in Social Work (MSW)
• Strong interpersonal and communication skills
• Compassionate, patient-focused approach to care
• Ability to work independently while collaborating with an interdisciplinary team
• Commitment to professionalism, integrity, and excellence in service

Responsibilities

• Provide psychosocial assessments and ongoing support to patients and families
• Assist with care planning and coordination of community resources
• Advocate for patient needs and goals
• Help patients and families navigate healthcare, financial, and social support systems
• Collaborate with an interdisciplinary care team to deliver exceptional patient-centered care
• Support patients and families through illness, recovery, and end-of-life journeys
