About The Position
John Knox Village is seeking a compassionate, licensed social worker to support the emotional, social, and overall well-being of home health patients. The Medical Social Worker works with patients, families, healthcare professionals, and community partners to help ensure patients receive appropriate resources, advocacy, and support. The role includes assessing patient needs, providing counseling and crisis support, coordinating care and community services, and maintaining compliant documentation. Requirements
- Licensed Medical Social Worker in the State of Florida.
- Strong communication and interpersonal skills.
- Ability to work independently while collaborating effectively with interdisciplinary teams.
- Knowledge of medical terminology, healthcare practices, and regulatory requirements.
- Experience working with diverse patient populations and providing culturally sensitive care.
- Level 2 background screening in accordance with Florida law.
Responsibilities
- Assess social, emotional, and environmental factors affecting patient health and well-being.
- Develop and implement individualized social service care plans.
- Provide counseling, emotional support, and crisis intervention for patients and families.
- Connect patients with community resources, support services, and benefit programs.
- Advocate for patients' rights and help them navigate healthcare systems.
- Facilitate support groups and educational programs.
- Participate in interdisciplinary care conferences and collaborate with healthcare teams.
- Coordinate services with external agencies and community organizations.
- Maintain accurate, timely, and compliant documentation of patient interactions and services.
