Requirements

A heart to serve patients and families and a passion for providing the best possible care
MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
Current state license as a social worker
2+ years of clinical work experience
Reliable transportation
Ability to sit, stand, bend, move intermittently and lift at least 25lbs and bear the weight of an average adult effectively

Nice To Haves

Clinical work experience preferably in healthcare or hospice

Responsibilities

Conduct psychosocial evaluations of patients and families
Provide ongoing counseling to patients and families during their end-of-life journey
Work in accordance with the plan of care
Provide emotional support to patients and families when it's needed most
