As a Home Health Medical Social Worker, you’ll provide compassionate, patient‑centered support in the comfort of patients’ homes, helping people and families navigate the emotional, social and practical challenges of illness and recovery. You’ll assess psychosocial needs, connect patients with community resources, support care planning and advocate for goals that promote dignity, stability and independence. Requirements

Master's Degree in Social Work.
Two (2) years of social work experience in health care setting.
Licensure as a Social Worker in the State of Connecticut.
