Requirements

Education: Master of Social Work (MSW)
Certifications: Shall have current Basic Life Support (BLS) by way of the American Heart Association or American Red Cross.
Experience: A minimum of two years full-time experience in care coordination, discharge planning or case management in a medical setting within the last four years.

Responsibilities

Oversee and manage the EFMP-M program in alignment with DoD and Air Force policy.
Identify and support families with special medical or educational needs using clinical assessments and collaborative outreach.
Ensure accurate enrollment (“Q coding”) and maintain data in Air Force special needs systems.
Train installation personnel and provide ongoing consultation on EFMP-M implementation.
Coordinate Family Member Relocation Clearance (FMRC) processes, enrollment, and travel screening.
Maintain up-to-date case files and conduct annual outreach to EFMP-enrolled families.
Collaborate with base agencies (IDS, MPS, CSS, AFPC) to ensure integrated support services.
Participate in quarterly case reviews and refer families to appropriate base or community resources.
Document clinical assessments and plans in GENESIS and maintain confidentiality in all records.
Support orientation and training of new staff and participate in relevant committees or task forces.
Maintain professional standards and compliance with DoD, Air Force, and facility-specific guidelines.
Complete documentation, data tracking, and required training in a timely manner.
