About The Position

Tryfacta is seeking a Social Worker for our client in Coxsackie, NY 12051. This is a 105-week contract assignment. The role involves providing individual therapeutic services to forensic mental health patients in a residential mental health unit. The Social Worker will serve as the primary therapist, participate in treatment planning and care coordination, and conduct comprehensive mental health assessments. Responsibilities also include completing suicide assessments, developing and updating treatment plans, preparing clinical documentation, developing discharge plans, and collaborating with multidisciplinary teams. Compliance with New York State Office of Mental Health (OMH) standards and participation in required training are essential. Requirements

  • Current New York State Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) license.
  • Experience providing mental health counseling, assessment, and case management services.
  • Knowledge of treatment planning, crisis intervention, suicide risk assessment, and behavioral health documentation.
  • Strong interpersonal, communication, and clinical interviewing skills.
  • Ability to work effectively with forensic, justice-involved, or high-acuity mental health populations.
  • Ability to collaborate effectively within a multidisciplinary team environment.
  • Successful completion of required TB testing and facility onboarding requirements.
  • Ability to complete CNYPC Orientation and comply with all facility security protocols.

Nice To Haves

  • Experience with electronic health records and clinical documentation systems preferred.
  • Prior experience in correctional, forensic, psychiatric, or residential mental health settings preferred.

Responsibilities

  • Provide individual therapeutic services to forensic mental health patients in a residential mental health unit.

  • Conduct comprehensive mental health assessments, including Core Histories and risk assessments.
  • Complete suicide assessments and ongoing evaluations of patient safety and well-being.
  • Develop, maintain, and update treatment plans in accordance with OMH policies and procedures.
  • Prepare accurate and timely clinical documentation, including weekly and monthly progress notes.
  • Develop discharge plans and coordinate continuity of care services.
  • Collaborate with multidisciplinary treatment teams to support patient recovery and rehabilitation.
  • Maintain compliance with New York State Office of Mental Health (OMH) standards, policies, and procedures.
  • Participate in required orientations, training programs, and facility-specific requirements.
