About The Position

Human Touch Home Health is seeking a compassionate Licensed Master Social Worker (LMSW) for PRN home health opportunities throughout Northern Virginia. We have patient needs across Fairfax, Loudoun, Prince William, Arlington, Alexandria, and Falls Church, with opportunities in communities including McLean, Vienna, Annandale, Springfield, Burke, Reston, Herndon, Chantilly, Centreville, Leesburg, Ashburn, Sterling, Manassas, Gainesville, Haymarket, Woodbridge, Dumfries, and surrounding areas. You do NOT have to cover all of Northern Virginia. Choose the cities or areas that work best for you and build your caseload around your availability. Sometimes what keeps a patient from succeeding at home has nothing to do with the clinical treatment plan. It may be transportation, food insecurity, caregiver burnout, financial concerns, housing challenges, or simply not knowing what resources are available. That's where your work matters. Requirements

Master's Degree in Social Work Required
Appropriate Virginia Social Work Licensure Required
Strong assessment, communication, and interpersonal skills
Knowledge of community resources and social service programs
Valid Driver's License and reliable transportation
Ability to work independently in patients' homes

Nice To Haves

Home Health or Healthcare Social Work Experience Preferred, but Not Required
WellSky Experience is a Plus

Responsibilities

Complete psychosocial assessments in patients' homes
Identify social, emotional, financial, and environmental barriers affecting care
Connect patients and families with appropriate community resources and support services
Assist with long-term care planning and available financial resources
Provide short-term counseling and emotional support
Help patients and caregivers navigate community programs and services
Advocate for patient needs and help address barriers to remaining safely at home
Collaborate with nurses, therapists, physicians, and other members of the care team
Participate in care and discharge planning
Complete accurate and timely documentation using WellSky
