About The Position

Human Touch Home Health is seeking a compassionate Licensed Master Social Worker (LMSW) for a PRN home health opportunity serving Blacksburg, Christiansburg, Radford, Pulaski, Floyd, and surrounding New River Valley communities. Home health patients and families often face challenges that go beyond their medical needs. As a Social Worker, you'll help connect them with resources, provide support, and remove barriers that may affect their ability to remain safely at home.

Requirements

• Master's Degree in Social Work from an accredited program
• Appropriate Virginia Social Work Licensure Required
• Valid Driver's License and reliable transportation
• Strong communication, assessment, and documentation skills
• Ability to work independently in patients' homes

Nice To Haves

• Home Health or Healthcare Social Work Experience Preferred
• WellSky experience is a plus

Responsibilities

• Complete psychosocial assessments in patients' homes
• Identify social, emotional, financial, and environmental needs
• Connect patients and families with community resources
• Assist with long-term planning and available support services
• Provide counseling and emotional support within scope of practice
• Assist patients and families with navigating healthcare and community programs
• Educate patients and caregivers on available resources
• Collaborate with nurses, therapists, physicians, and other care team members
• Document visits and interventions accurately and on time using WellSky
• Support the overall plan of care and patient goals
