We have an opening for a Licensed Master Social Worker to work Full Time. Lakeview's Licensed Master Social Worker promotes the psychosocial well-being of the residents by supporting resident-centered care. Assists in interpreting the social, psychological, and emotional needs of the residents and their families to the interdisciplinary team involved with the residents’ plans of care. Works closely with the interdisciplinary team to ensure that the residents live in a safe, comfortable, and functional environment. Ensures that all appropriate related documentation is maintained and compliant with Local, State, and Federal regulations.

Requirements

  • Must possess a Master’s Degree from an accredited school of Social Work and be licensed to practice in the State of Kansas.
  • Current CPR certification is required.
  • Must demonstrate strong interpersonal skills while working in a team-oriented setting.
  • Must demonstrate an effective understanding of Microsoft Office Suite software.
  • Ability to communicate effectively in English, including reading, speaking, and writing, when performing duties that require this need, and with individuals from diverse backgrounds.
  • Possesses an empathetic approach to the elderly.
  • Able to work in a team setting with residents, relatives, staff, and others.

Responsibilities

  • Ensures the completion of PASRR screening, initiates the CARE assessment process with KDADS and the Area Agency on Aging.
  • Schedules and performs admission assessments and annual assessments, ensures that care plans and CAA documentation are completed in regards to residents’ social history, cognition, signs of depression, and other social service needs.
  • Assists with determining which services are eligible/covered under residents’ HMO insurance or Medicare; assists with Part B authorizations, LTC insurance updates, and applications for Medicaid.
  • Assists as needed in completing documentation for financial resources such as LTC insurance, Medicaid, Veteran’s benefits, etc.
  • Assists in developing a specific written plan of care for each resident.
  • Attends and participates in scheduled care plan meetings.
  • Ensures that residents are offered the opportunity to complete all necessary documents such as power of attorney and living will in order to direct decisions that may be made on their behalf; encourages and involves the residents’ families in planning Social Service programs when possible.
  • Facilitates hospice referrals as appropriate, in coordination with the interdisciplinary team, resident, and their family.
  • Coordinates both on- and off-campus podiatry, dental, and optometry appointments for residents.
  • Ensures that compassionate relationships are fostered through shared-living activities and that the psychosocial, emotional, and spiritual needs of residents and staff members are fulfilled.
  • Provides vision and leadership to the neighborhood by displaying openness and adaptability to change and new ideas.
  • Assists in developing, planning, and implementing discharge procedures.
  • Provides consultation to other staff members in efforts to solve problems and meet needs of residents through Social Service programs.
  • May be asked to supervise Social Services designees or Bachelor Level Social Workers.
  • May act as the Practicum Supervisor of Bachelor and Master Level Social Work interns to facilitate a positive, meaningful learning experience.
  • Assists with serving in the Dining Room according to resident diet profiles.
  • Assists in cleaning common spaces, dining areas, and resident according to established procedure.
  • May be assigned Homemaker duties as needed.
  • Develops and maintains a good working rapport with inter-department personnel, other departments within the facility, and off-campus contacts to ensure that operations consistently meet the needs of the facility and residents.
  • Ensures that Resident’s Rights to fair and equitable treatment, self-determination, individuality, privacy, property, and civil rights, including the right to express grievances are followed.
