About The Position

The Social Worker assesses psychological, social, emotional, cultural, and/or financial needs that affect patients’ health and recovery. The role provides professional social work services, including supportive counseling and clinical assessment of patients’ and families’ needs during illness, crisis intervention, and post-discharge planning. The Social Worker collaborates with the healthcare team to develop transition plans and address needs in a timely manner, and participates in program planning and development to meet the needs of customers in the healthcare system and community.

Requirements

• Certified LMSW.
• Missouri Social Work license.
• Master’s degree in Social Work.

Nice To Haves

• Hospital social work experience is preferred.

Responsibilities
