About The Position

Do you desire to make a difference in your community and be off on most evenings and weekends? If so, this is the job for you!! This is a full-time position located in Albany, Georgia, within the Southwest Public Health District 8-2/Infectious Disease. The role involves providing a broad range of social services to individuals with HIV/AIDS and their families, assessing eligibility for the Ryan White Program, developing plans, linking clients to community resources, and maintaining data for reporting. While the role aims for evenings and weekends off, there may be instances requiring work during these times for special events or public health emergencies.

Requirements

  • Bachelor's degree in a related field from an accredited college or university
  • Two years of experience in social service delivery.
  • Must be able to lift up to 45 lbs, stoop, and bend.
  • Valid driver's license required

Responsibilities

  • Provides outstanding customer service
  • Maintains HIPAA standards related to Privacy and Security
  • Provides a broad range of social services to individuals with HIV/AIDS and their families.
  • Assesses to determine eligibility for the Ryan White Program and enrolls and recertifies as appropriate.
  • Develops plans and assesses effectiveness.
  • Links to community resources for assistance.
  • Participates in community consortium meetings.
  • Collects and maintains data and completes and submits required reporting
  • May work nights and weekends, participate in health fairs, serve on committees, and make presentations, or respond to disasters.
  • Responds to Public Health Emergencies
