The Social Worker will address psychosocial factors relevant to diagnosis, illness, or hospitalization through assessment, counseling, community relationship building, social service coordination, education, research, and teaching within the Neurodevelopmental Health program.

Requirements

  • Master’s of Social Work degree
  • Membership in the Ontario College of Social Workers and Social Service Workers
  • Minimum three (3) years of relevant experience
  • Comprehensively use computer technology
  • Take steps to ensure timely completion of tasks by adjusting priorities as required
  • Adapt to changes to meet patient needs
  • Communicate effectively and concisely, both orally and in writing
  • Deal effectively and sensitively with patients and/or relatives to obtain cooperation and approval of actions
  • Deal effectively in crisis or traumatic situations involving extreme emotional situations
  • Deal effectively with external customers of considerable importance
  • Working knowledge of applicable occupational health and safety legislation; general knowledge of corporate/program policies and procedures related to health and safety
  • Function well in an interdisciplinary team
  • Apply knowledge of child protection issues
  • Access to Vehicle and Valid Driver’s License
  • Vulnerable Sector Check (Level 3) and remain compliant
  • Bilingualism (French/English)

Responsibilities

  • Assess family dynamics, functioning, strengths, and needs.
  • Provide emotional support and counseling (e.g., trauma, crisis intervention, adjustment, behavior management).
  • Participate in interdisciplinary team meetings, contributing expertise on psychosocial and emotional needs.
  • Provide brief psychotherapy to individuals, groups, and families.
  • Conduct ongoing assessments of psychosocial factors impacting child well-being.
  • Provide occasional after-hours response for new diagnoses and complex situations.
  • Liaise with community agencies to establish resources and services for children and families within the Autism Program.
  • Participate in the development and implementation of services and programs to address psychosocial needs.
  • Maintain current and accurate psychosocial documentation.
  • Consult and collaborate with various community agencies (e.g., ministries, school boards, child welfare authorities, police).
  • Identify, advocate for, negotiate, and obtain community resources for children and families.
  • Assist with discharge planning and coordinate services within the hospital and community (e.g., home/school visits).
  • Participate in hospital committees (e.g., Social Work, Family Forum, PLEO, Community Advisory Committee).
  • Complete daily statistical documentation (e.g., GRASP).
  • Supervise and train social work students (BSW and MSW levels) and provide practicum opportunities.
  • Contribute to the education/training of other professionals, students, and volunteers regarding social needs of the ASD population.
  • Present workshops within the hospital and community.
  • Promote understanding of topics like chronic illness, mental health, and child protection through lectures.
  • Participate in panel discussions and educational rounds.
  • Participate in committee work such as Quality Management, Policy and Planning, and Accreditation.
  • Promote social awareness in the community by participating as a board member, educator, conference speaker, consultant, and expert witness.
  • Participate in research and develop/present special projects (e.g., respite care, child abuse, and neglect).
