About The Position
The Social Worker promotes patient and family wellness, improves care outcomes, and helps patients access appropriate hospital and community resources. The role manages, supports, and develops comprehensive transitional care plans for patients with complex and psychosocial needs in inpatient and Emergency Department (ED) environments.
Requirements
- Master’s degree from an accredited social work program.
- No experience required.
Nice To Haves
- Two years of experience in an acute or post-acute setting.
- Licensed Master Social Worker (LMSW) in the State of Georgia.
