The Social Worker (MSW) provides non-clinical biopsychosocial assessments and interventions for inpatient units and the outpatient emergency department. The role includes crisis intervention, patient and family interventions, high-risk screening, focused emotional support, and referrals for financial assistance or other resources. The MSW assists with post-acute care placement for discharges, particularly for patients with behavioral health, assistance, or resource needs after discharge, and responds to physician and other non-discharge-planning social work consultations throughout the facility. The role serves areas including intensive care, Trauma, Oncology, NICU, Transplant, and the ED, with a focus on patient- and family-centered care, self-determination, and compassionate problem-solving. Working collaboratively with RN case managers, acute care nurses, physicians, insurers, and other staff, the MSW identifies patient needs and barriers to discharge and recovery. The position assesses high-risk patients, develops discharge plans, coordinates care meetings, supports patients and families in difficult decisions, and provides consultation, education, counseling, advocacy, and referrals addressing emotional, social, and financial needs. The MSW also provides support during complex or emotionally charged situations, including end-of-life care, terminal illness and death notifications with a physician, and crisis interventions.
