About The Position

The Social Worker LMSW collaborates with an interdisciplinary team to assist patients and families with complex psychosocial needs. Interventions may include alternate-site coordination, discharge planning, addressing identified abuse or neglect, adoption, substance use disorders, end-of-life decisions, referral resources, bereavement support, adjustment counseling, guardianships, and behavioral health concerns, including psychiatric transfers. The role also provides short-term therapeutic support appropriate to the setting and location.

Requirements

• Master’s degree in Social Work.
• Michigan Master Social Worker license required upon hire.
• Michigan driver’s license required upon hire for Hospice and Home Care.

Nice To Haves

• Two years of relevant healthcare experience after obtaining a Master’s degree in Social Work.
• Care management experience.
• Experience in individual and family assessment, crisis intervention, grief and loss counseling, and discharge planning.

Responsibilities

• Conduct psychosocial assessments with patients and/or families to identify needs, including potential alternate sites of care after discharge, and develop and execute a plan.
• Communicate effectively and promptly with alternate sites of care and community agencies to address patient needs.
• Complete and document assessments and plans in the electronic medical record, accurately reflecting the patient’s condition, circumstances, transition-of-care needs, and psychosocial priorities.
• Provide consultation and resources to healthcare team members.
• Maintain current knowledge of relevant community resources.
• Participate in or assist with department, regional, or statewide stakeholder committees and projects as assigned or requested.
• Develop and maintain current knowledge of federal and state regulations relevant to the role.
• Provide short-term therapeutic support appropriate to the setting and location.
