Monitors and provides comprehensive psychosocial care in accordance with accepted standards of practice of the National Association of Social Workers (NASW) and established EMC policies and procedures. The Social Worker will conduct comprehensive biopsychosocial assessments, facilitate behavioral change, provide social and emotional support, and coordinate care transitions. They will also conduct screenings, provide care management services, and build patient self-advocacy skills. Collaboration with the interdisciplinary team is essential for process improvement and patient care.
