About The Position

Dignity Health Connected Living provides programs and affordable health care services for seniors and people with disabilities in Shasta County. The Social Worker serves as a compassionate advocate for individuals and families facing complex challenges, providing assessment, care planning, counseling, crisis intervention, and connections to community resources. The role collaborates with multidisciplinary teams to provide holistic support, promote well-being, and empower clients.

The Social Worker assesses participants, develops and implements appropriate care plans, conducts clinical evaluations, and diagnoses mental illness. The position provides support and communication to the social service department, exercises judgment about treatment, and delivers mental and behavioral therapy using a strength-based social work approach. Counseling may involve non-medical applied psychotherapy with individuals or groups, direct therapy, referrals, and peer support groups.

Requirements

• Master of Social Work (MSW) in Counseling Psychology.
• Two years of experience working with elderly people or disabled adults.
• Experience conducting interviews, assessing clients, developing and implementing care plans, and working with groups and community agencies.
• California Licensed Clinical Social Worker (LCSW).
• Exceptional interpersonal skills, empathy, knowledge of community resources, and crisis management abilities.

Nice To Haves

• MFT or MA in Counseling or Psychology.
• Coursework in gerontology.

Responsibilities
