About The Position

The Social Worker LCSW supports dialysis patients and their families through orientation and psychosocial assessment, screening, and care planning. The role also participates in facility quality improvement activities and may serve as an alternate facility representative to the ESRD Network. Duties described are general in nature and may include additional responsibilities as delegated.

Requirements

• Hold a master's degree in social work with a specialization in clinical practice from a school of social work accredited by the Council on Social Work Education.

Responsibilities

• Orient new patients and their families to dialysis.
• Complete the initial psychosocial component of the Interdisciplinary Assessment within 30 days of admission, a subsequent assessment at 90 days, and annual updates; file assessments in the patient's medical record.
• Complete a KDQOL, clinical depression screening, and monthly care plan meeting.
• Act as an alternate facility representative to the ESRD Network.
• Attend the monthly quality assessment and performance improvement process meeting.
