About The Position

Under general supervision, provides a full range of direct professional social work services to specialized populations such as children, seniors, substance abusers, and physically or mentally ill individuals. This role involves providing case management services for individuals referred to the Case Management Program, assessing, evaluating, and monitoring individual cases for service planning and treatment coordination. The position assists in rendering diagnosis and developing individual treatment/service plans designed to prevent or reduce health, social, or environmental challenges, and assists with timely and coordinated access to medical appropriate levels of health and support services. The Social Worker II also provides counseling and education to promote treatment adherence and support client engagement, consults with interdisciplinary health or social work professionals, and serves as a liaison across health, social services, and community agencies to ensure continuity of care. Additionally, the role evaluates client eligibility for emergency financial assistance, provides advocacy, resource navigation, referrals, and linkage to community supports, crisis intervention, and ongoing monitoring of client progress. Responsibilities include preparing case histories and reports, maintaining accurate client records, and participating in public awareness and education programs. Other duties may include making home visits, transporting clients, assisting senior adults in emergencies, and attending training.

Requirements

  • Must possess at least a master’s from a college or university approved by the Council of Social Work Education.
  • Minimum of 2 years' experience rendering professional social work services.
  • Possession of a license, issued by the Maryland Board of Social Work Examiners, as a Licensed Certified Social Worker (LCSW) is required at the time of application.
  • Some positions require possession of a LCSW-Clinical license issued by the Maryland Board of Social Workers at the time of application.
  • Knowledge of the principles, practices, and theories of social work and human behavior.
  • Knowledge of the diagnosis, treatment, and management of social, emotional, and mental dysfunctions.
  • Knowledge of current social, economic, and health problems.
  • Knowledge of individual, family, and group therapy dynamics and techniques.
  • Knowledge of social work programs, and program evaluation methods.
  • Knowledge of alternate and additional resources for clients/patients.
  • Skill in applying social work theories and principles to individuals in a social work program.
  • Skill in the development of effective treatment or service programs for individuals and their families and in providing crisis intervention.
  • Skill in leading goal-oriented and long-term individual, family, and group counseling or therapy sessions.
  • Skill in interpreting and applying Federal, State, and local guidelines, regulations, and procedures.
  • Skill in preparing accurate and complete, forms, case histories, and reports.
  • Skill in establishing rapport with clients/patients.
  • Good interpersonal skills.
  • Ability to establish effective working relationships with all contacts.
  • Ability to assist in the evaluation of programs and services.
  • Ability to instruct and to set standards for assigned program and services.
  • Ability to review and coordinate the work of others.
  • Knowledge of applicable Federal, State, and local regulations, procedures, and guidelines.
  • Knowledge of County and departmental rules, regulations, policies, and procedures relating to the performance of duties.

  • Some positions may require satisfactory completion of County sponsored CPR and First Aid training.
  • Employees must maintain licenses and certifications throughout the course of their employment.
  • Some positions in this classification may be required to respond to emergency situations and work overtime with little or no notice.
  • Some positions require employees to complete an approved training program and must pass a competency test by the second training available after appointment to this classification.

Nice To Haves

  • Education beyond a master's degree from an accredited college or university in the field of social work may be substituted on a year-for-year basis, up to a maximum of one year for the required experience.

Responsibilities

  • Serves as a liaison across health, social services, and community agencies to ensure continuity of care.
  • Evaluates client eligibility for emergency financial assistance based on established criteria.
  • Provides advocacy, resource navigation, referrals, and linkage to community supports.
  • Provides crisis intervention and individual or group counseling to clients.
  • Conducts ongoing monitoring of client progress, stability, and service needs.
  • Prepares case histories and other reports, memos, and correspondence in accordance with program standards.
  • Maintains accurate, timely records of client activities, services, and outcomes.
  • Participates in public awareness and education programs.
  • Makes home visits and evaluates living/environmental conditions.
  • Transports patients/clients.
  • Assists sick or injured senior adults until medical personnel arrive at the scene.
  • Attends training and seminars.
  • Performs other duties as assigned.
