0000 Facility - Social Worker I- PRN $27-$30 an hour
About The Position
Plan, direct, define, and coordinate individualized and group therapeutic support and services for patients with mental health, behavioral health, and substance use issues. The role conducts social history assessments, helps develop and implement treatment plans, coordinates programs and activities, supports patient recovery and social functioning, and provides crisis management and family sessions. The Social Worker participates in multidisciplinary treatment and discharge planning, maintains patient documentation, facilitates groups and referrals, and responds professionally to patient needs while prioritizing safety. Other functions and tasks may be assigned.
Requirements
- Bachelor’s degree in Social Work from an accredited college or university; new graduates are eligible.
- Eligible for state licensure as a Licensed Social Worker (LSW), Licensed Associate Counselor (LAC), Certified Addictions Counselor (CAC), or Registered Addiction Counselor (RAC), as required by the state where the facility operates.
- CPR and de-escalation/restraint certification required; training is available upon hire and offered by the facility.
Nice To Haves
- One or more years of experience working with the patient population is preferred.
- First aid may be required depending on the state or facility.
Responsibilities
- Conduct social history assessments through observation and patient or family interviews, gathering information about patient needs, skills, interests, and limitations to support treatment planning.
- Coordinate and plan programs and activities to meet patients’ social and emotional needs.
- Promote recovery, symptom reduction, and the highest attainable level of social functioning.
- Provide crisis management and family sessions to help families understand life situations and their effects on lifestyle.
- Assist in developing effective treatment plans that appropriately apply therapeutic principles while maintaining patient safety.
- Participate in multidisciplinary treatment team meetings to discuss patient assessments, treatment plans, and ongoing discharge plans.
- Communicate with or assist family members in understanding and supporting patients.
- Manage a caseload and maintain patient chart documentation.
- Facilitate education, process, and support groups, and provide referrals as requested.
- Maintain a positive, empathetic, and professional attitude toward customers; acknowledge and work to resolve unmet patient needs or complaints, with patient safety as a priority.
- Perform other functions and tasks as assigned.
