About The Position

This role involves leading the design and execution of personalized care plans, ensuring every step from admission to discharge reflects the patient’s unique values and recovery goals. The Social Worker will advocate for patients by identifying and removing social or financial barriers to care, ensuring they have the resources needed for long-term health. They will also support families through complex psychosocial challenges, offering expert guidance during crises, substance abuse recovery, or difficult life adjustments. Collaboration with the interdisciplinary care team is key to refine treatment strategies, ensuring seamless communication and high-quality medical documentation that drives better clinical outcomes.

Requirements

Social Worker credentialed from the Texas State Board of Social Worker Examiners obtained prior to hire date or job transfer date required.
Bachelor's degree in Social Work required.
Active State Licensure: Current and unrestricted license in Social Work (LMSW/LCSW) as required by state regulations.
Clinical Case Management Experience: A proven track record of coordinating complex discharges and care transitions within an acute care or clinical setting

Nice To Haves

Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
3 years of experience preferred.
