About The Position

This role focuses on guiding seamless patient transitions by creating individual care plans that blend clinical needs with patient and family values. The Social Worker will resolve critical psychosocial barriers as a subject-matter expert in crisis intervention, legal support, substance abuse, and adjustment needs to protect patient well-being. They will advocate for essential resources by proactively identifying community partnerships and systematically removing obstacles to post-discharge care. Additionally, the role involves partnering with interdisciplinary teams through continuous communication to evaluate health plans, optimize medical record documentation, and ensure every patient receives safe, high-quality care transitions.

Requirements

Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Nice To Haves

Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.
