Requirements

Licensed Clinical Counselor credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
Social Worker credentialed from the Wisconsin Board of Marriage & Family Therapy, Professional Counseling & Social Work obtained prior to hire date or job transfer date.
High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Nice To Haves

Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred.

Responsibilities

Design personalized care journeys across the full continuum of care, integrating patient and family preferences to ensure compassionate, values-based outcomes.
Serve as a trusted expert in crisis and psychosocial support, evaluating complex needs—from substance abuse to legal or adjustment challenges—and creating tailored treatment recommendations.
Fiercely advocate for patients by identifying systemic barriers and securing critical healthcare and community resources to enable smooth care transitions.
Drive cross-functional team alignment through continuous dialogue with supervisors and care transition teams, ensuring health plans are proactively evaluated and adapted.
Guide post-discharge success and documentation excellence by advising care teams on accurate medical records, complex levels of care, and diagnostic-driven treatment options.
