About The Position

This role involves coordinating the overall interdisciplinary plan of care across the continuum from admission to discharge, integrating patient/family preferences and values. The Social Worker will monitor patient care through assessments, evaluations, and medical records, acting as a resource for adequate documentation and appropriateness of services. Additionally, they will advocate for necessary resources, the removal of barriers, and appropriate treatment options for post-discharge care, while maintaining ongoing dialog with supervisors and other health providers to ensure the effective implementation of the health plan.

Requirements

  • Clinical Counselor credentialed from the Illinois Department of Financial and ProfessionalRegulation obtained prior to hire date or job transfer date.
  • Counselor credentialed from the Illinois Department of Financial and Professional Regulationobtained prior to hire date or job transfer date.
  • Social Worker specializing in Clinical Practice credentialed from the Illinois Department ofFinancial and Professional Regulation obtained prior to hire date or job transfer date.
  • Social Worker credentialed from the Illinois Department of Financial and Professional Regulation obtained prior to hire date or job transfer date.
  • High School diploma equivalency with 2 years of cumulative experience OR Associate'sdegree/Technical degree OR 4 years of applicable cumulative job specific experience required.
  • Previous in patient discharge experience needed.

Responsibilities
