Constellation Health is looking for a compassionate Licensed Medical Social Worker/Bereavement Coordinator to work with hospice patients and their families in their homes. As a social worker employed through the organization, they are responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement. In the Bereavement Coordinator role, the individual is responsible to plan, implement, and maintain a bereavement program to meet the needs of families/caregivers from Constellation Hospice for up to one (1) year following the death of the individual hospice patient.
