About The Position

The Bereavement Coordinator is responsible for planning, implementing, and maintaining a bereavement program to meet the needs of families and caregivers of CovenantCare Hospice patients for up to one year following the patient's death. This role involves coordinating the assessment and delivery of grief counseling services, including one-to-one sessions, group support, and ongoing follow-up, to ensure timely and appropriate care. The coordinator also provides bereavement information and referral services to the community, maintaining an updated resource list. Additionally, the position supports hospice personnel by offering one-to-one counseling for work-related grief, supervising interns providing counseling, and referring staff to community resources. The role includes participation in hospice activities, quality improvement teams, and assisting in planning support services for hospice personnel, such as retreats and support groups. The Bereavement Coordinator functions independently but seeks guidance from the interdisciplinary group and community members.

Requirements

  • Master's degree in Social Work
  • Qualifications of experienced clinical grief counseling
  • Experience with families/caregivers and patients through death/dying issues
  • Experience with clinical grief counseling
  • Understanding of grief response and experience with counseling individuals and/or in groups regarding loss.
  • Ability to work as a team member.
  • Knowledge and commitment to hospice philosophy.
  • Demonstrates good verbal and written communication, and organization skills.
  • Mobility to move around from one area to another.
  • Ability to lift loads not normally exceeding medium weight (35 pounds).
  • Manual dexterity, keen vision and hearing accuracy are necessary in driving a vehicle.

Responsibilities

  • Plan, implement, and maintain a bereavement program for families/caregivers of CovenantCare Hospice patients for up to one year following the death of the individual hospice patient.
  • Coordinate the assessment and delivery of grief counseling needs and services (one-to-one, groups, and ongoing follow-up) to ensure timely and appropriate services are provided to family/caregivers.
  • Provide bereavement information and referral services to callers from the community.
  • Expand and maintain community resource list as needed.
  • Participate in hospice activities such as in-service education, hospice personnel meetings, and relevant committees.
  • Provide bereavement support to hospice personnel coping with work-related grief through one-to-one counseling, supervision of interns providing one-to-one counseling, and referral to community resources.
  • Assist as needed in evaluating and planning services to meet hospice personnel support needs (i.e., yearly retreats, weekly support groups).
  • Function independently, but seek and accept guidance from other members of the interdisciplinary group, or from members of the community.
  • Actively participate in quality assessment performance improvement teams and activities.
