About The Position

The Bereavement Coordinator is responsible for planning, implementing, and maintaining a bereavement program to meet the needs of families and caregivers of CovenantCare Hospice patients for up to one year following the patient's death. This role involves coordinating the assessment and delivery of grief counseling services, including one-to-one sessions, group support, and ongoing follow-up, to ensure timely and appropriate care. The coordinator also provides bereavement information and referral services to community callers, expands and maintains a community resource list, and participates in hospice activities such as in-service education and personnel meetings. Additionally, the position supports hospice personnel coping with work-related grief through individual counseling, supervision of interns, and referrals to community resources, and assists in evaluating and planning services for personnel support needs. The role requires functioning independently while seeking guidance from the interdisciplinary group and community members, and actively participating in quality assessment and performance improvement activities.

Requirements

  • Master's degree in Social Work
  • Qualifications of experienced clinical grief counseling
  • Experience with families/caregivers and patients through death/dying issues
  • Experience with clinical grief counseling
  • Understanding of grief response
  • Experience with counseling individuals and/or in groups regarding loss
  • Ability to work as a team member
  • Knowledge and commitment to hospice philosophy
  • Demonstrates good verbal and written communication skills
  • Demonstrates good organization skills
  • Mobility to move around from one area to another
  • Ability to lift loads not normally exceeding medium weight (35 pounds)
  • Manual dexterity, keen vision and hearing accuracy necessary in driving a vehicle

Responsibilities

  • Plan, implement, and maintain a bereavement program for families/caregivers of hospice patients.
  • Coordinate assessment and delivery of grief counseling needs and services (one-to-one, groups, ongoing follow-up).
  • Provide bereavement information and referral services to community callers.
  • Expand and maintain community resource list.
  • Participate in hospice activities such as in-service education, personnel meetings, and committees.
  • Provide bereavement support to hospice personnel coping with work-related grief.
