Under general supervision of a Program Manager and within state and locally developed programs and policies, the social worker assesses individual, family and community challenges, barriers, and risk, and determines methods or services to work toward rehabilitation, prevention or management of, mental health, substance use, physical health and vulnerable adult issues. This position will conduct assessments for adults referred to or seeking adult services and supports within the Adult Behavioral Health Unit and /or community providers. The worker must be committed to evidenced based practices, demonstrate collaborative practices with families and professionals and demonstrate a working knowledge of adult behavioral health interventions.
