This position provides social work services to adults receiving mental health treatment, including admission assessments, direct patient contact, and communication with community providers, county liaisons, and family members. The role also includes offering social work consultation to other professional disciplines and the treatment team to support coordinated care.

Requirements

  • A bachelor's or master’s degree in social work or social welfare.
  • Applicants will be considered to have met the educational requirements once they are within 3 months of graduating with a qualifying degree.
  • You must meet the PA residency requirement.
  • You must be able to perform essential job functions.
  • A conditional offer of employment will require a medical examination and drug screening.
  • This position falls under the provisions of the Older Adult Protective Services Act. Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.

Responsibilities

  • Interview individuals, family members, guardians, and agency representatives to gather and update personal and social information
  • Complete psychosocial assessments and share findings with the treatment team to guide planning
  • Help develop and carry out individual treatment plans and ensure that needed services and benefits are obtained
  • Maintain case files, document contacts, and prepare reports in accordance with established procedures
  • Prepare petitions, assist with legal matters, coordinate with courts and probation, and participate in required hearings
  • Plan discharges, complete summaries, and work with community agencies to support safe transitions
