About The Position

Provides comprehensive social work services, including discharge planning and coordination, patient education, and counseling. Helps meet the psychosocial and discharge-planning needs of patients and families and enables patients to utilize medical care to achieve their optimal level of health.

Requirements

• Master's degree in Social Work.
• New York State license in Social Work.
• At least one year of experience as a Social Worker.
• Discharge planning experience.
• Knowledge of Medicare regulations.
• Understanding of the managed care environment.
• Excellent interpersonal and communication skills.

• Case Manager Certification.
