About The Position
Enhabit Home Health & Hospice is seeking a Licensed Master Social Work professional to administer social services to patients who require intermittent professional services. The role also involves teaching patients, families, and other members of the health care team. Services must be provided in accordance with physician orders and the established plan of care.
Requirements
- Must have a master’s degree from a school of social work accredited by the Council on Social Work Education.
- Must be certified or licensed in the state where currently practicing.
- Must have at least one year of social work experience in a health care setting.
- Must have basic demonstrated technology skills, including operation of a mobile device.
- Must possess a valid state driver license.
- Must maintain automobile liability insurance as required by law.
- Must maintain dependable transportation in good working condition.
- Must be able to safely drive an automobile in all types of weather conditions.
- Must possess CPR certification.
- State-specific education and licensure requirements vary by state; requirements listed include specified degrees and social work licenses or certifications, or working under an appropriately credentialed social worker.
Nice To Haves
- Medicare home health or hospice experience is preferred.
