About The Position

The Social Services Director identifies and provides for each resident’s social, emotional, and psychological needs, supports the continuing development of each resident’s full potential during their stay, and assists with discharge planning. The role is based at Signature HealthCARE at Summerfield Rehab & Wellness Center, a 165-bed facility providing short-term rehabilitation and long-term care.

Requirements

• Master’s degree in Social Work or Psychology.
• At least 3 years of experience working in Social Services directly with individuals, preferably in a healthcare setting.
• High level of professionalism and ability to maintain confidentiality.

Responsibilities

• Assist residents and their families with social, emotional, and family issues.
• Complete required documentation accurately and on time to comply with facility, state, and federal policies.
• Participate in resident care planning.
• Participate in discharge planning and prepare residents for the transition.
• Serve as a general source of information for residents and their families.
