The Social Services Director is responsible for handling pressure, objectivity, confidentiality, organization, planning, reporting skills, persistence, being proactive, listening, verbal communication, and managing resident and family relationships. This role involves establishing courses of action by exploring options and setting goals with residents and their families. The director will assist residents in achieving the highest practicable level of self-care, independence, and well-being by providing medically related social services. This includes evaluating social and family information, psychological and emotional needs, developing care plans, providing emotional support, addressing emotional problems, assisting residents and families with coping mechanisms for physical disabilities, health-related fears, and grief. The role also involves assisting residents with healthcare decisions, personal shopping, inventory and tracking of patient belongings, coordinating responses to missing items, and securing community resources. Additionally, the director will foster strong working relationships with other departments and outside agencies, document resident social service status, assist with outpatient and ancillary patient services, and coordinate discharge planning with relevant parties. The position also includes providing in-service training to staff and counseling residents and family members.
