About The Position

Helia Healthcare is seeking a Social Services Director to support residents’ practical, emotional, and social needs. The role assesses residents’ psychosocial needs and risks, develops interventions to improve quality of life, and completes required documentation. The director coordinates safe discharge plans, leads the social services team, coordinates the Family Council, manages grievances, and collaborates with families, physicians, staff, and referral sources. The role also tracks quality indicators.

Requirements

• Bachelor’s degree in social work or a related human-services major, such as sociology, gerontology, psychology, rehabilitation counseling, or special education.
• At least one year of social work experience with supervisory duties and direct work with geriatric residents.
• Current Missouri or Illinois Social Work license, depending on location.
• Experience with Medicaid and Medicare.
• Regulatory knowledge.
• Leadership and communication skills.
• Ability to collaborate with physicians and other healthcare professionals.
• Working knowledge of behavioral health systems.
• Networking skills.

Responsibilities
