About The Position

Williamsburg Village Healthcare Campus in DeSoto, Texas, is hiring a Social Services Director for its long-term and rehabilitative care facility. The director is a member of the facility’s interdisciplinary team and participates in care planning, coordinates referrals to other healthcare providers, and facilitates development of resident discharge plans. The role also coordinates self-governing resident and family councils, admissions and discharges, and may handle resident and family grievances while serving as a resident advocate when needed.

Requirements

Bachelor’s degree in social work.
Current Social Worker license from the state of practice.
At least one year of supervised experience in a healthcare setting working directly with clients or residents.
Current knowledge of state and federal laws applicable to social work in long-term care.
Excellent interpersonal and communication skills.

Skilled nursing facility experience preferred.

Responsibilities

Participate in care planning as part of the facility’s interdisciplinary team.
Coordinate referrals to other healthcare providers.
Facilitate the development of resident discharge plans.
Coordinate self-governing resident and family councils.
Coordinate all facility admissions, including readmissions, room moves, and patient inquiries.
Coordinate all discharges and prepare written discharge plans that include family and community services as appropriate.
Coordinate resident and family member grievances when required.
Act as the resident advocate when necessary.
Plan, organize, and implement social service programs for residents with varying needs.
