About The Position

The Social Services Director oversees the development, implementation, supervision, and ongoing evaluation of the Social Services Department to help residents attain or maintain their highest practicable well-being. The role identifies residents’ medically related social service needs and ensures services are provided in accordance with State and Federal regulations. The director completes or delegates the social services portion of comprehensive assessments and contributes or delegates contributions to social services goals and approaches in care plans. The director oversees preadmission services for incoming residents, coordinates interdisciplinary discharge planning to support safe transitions back into the community, and arranges community-based services and follow-up care. The role also oversees the establishment and appropriate modification of departmental quality assurance procedures.

Requirements

A bachelor’s degree in social work or another human services field, including sociology, gerontology, special education, rehabilitation counseling, or psychology.
One year of supervised social work experience in a health care setting working directly with individuals.
A social worker license in the required state.

Responsibilities

Identify residents’ need for medically related social services and ensure services are provided in accordance with State and Federal regulations.
Complete or direct/delegate completion of the social services component of the comprehensive assessment.
Contribute or direct/delegate contributions to social services goals and approaches in residents’ care plans.
Oversee preadmission services for incoming residents.
Facilitate residents’ safe transition back into the community through interdisciplinary discharge planning and arrangement of community-based services and follow-up care.
Oversee the establishment of departmental quality assurance procedures and modify them where appropriate.
