Social Services Designee
About The Position
The Social Services Designee coordinates the overall interdisciplinary plan of care for residents, from admission to discharge. You will act as a liaison between resident/family and healthcare personnel to ensure necessary care is provided promptly and effectively. Requirements
- Related coursework in social work or psychology
- Able to read, write and communicate in English

- Hospital or nursing home experience, preferred
Responsibilities
- Coordinates the overall psychosocial care plan for residents.
- Provides support and advocates for resident/patient and their family.
- Develops and maintains community resources for residents.
