Responsibilities
- Conducting psychosocial assessments
- Navigating the complex Medicare/Medicaid resources alongside families
- Participating the team care planning process
- Coordinating safe discharge plans
- Resident and family counseling & support
- Regulatory compliance and documentation
- Advocacy and grievance interventions
