About The Position

Royal Oaks in Duarte, California, is seeking a Temporary Social Services Designee to join its Health Center team. Under minimal supervision, the Social Services Designee assesses residents’ social service needs and ensures identified needs are addressed. The role advocates for residents and families while maintaining a professional, positive customer service approach.

The schedule is Monday through Friday, 8:00 a.m. to 4:30 p.m. Compensation is $30.00–$35.00 per hour, depending on experience (DOE).

Requirements

• Minimum of two years of experience working with older adults in a long-term care setting.
• Strong communication, problem-solving, and interpersonal skills.
• Knowledge of community resources.

Nice To Haves

• Bachelor’s degree in Social Work preferred.
• Social Services certification desired.

Responsibilities

• Assess residents’ social service needs and provide compassionate support to enhance their well-being and quality of life.
• Coordinate resident care plans, admissions, discharges, and transitions in collaboration with the interdisciplinary team.
• Support residents and families by addressing concerns, resolving grievances, and connecting them to appropriate community resources.
• Maintain accurate social service assessments, progress notes, and documentation.
• Advocate for residents’ rights and individual needs.
