About The Position

The Social Service Designee assists with planning, developing, organizing, implementing, evaluating, and directing the facility’s social service programs in accordance with applicable federal, state, and local standards and facility policies and procedures. The purpose is to help ensure residents’ medically related emotional and social needs are met and maintained on an individual basis. The role carries delegated administrative authority, responsibility, and accountability for assigned duties. The Center at Waterfront is a rehabilitation and complex nursing care facility. Its care includes skilled nursing, family involvement, psychological care, and discharge planning to support residents’ individual needs and functional goals.

Requirements

Must possess a Social Service Designee certificate.
Must be able to read, write, speak, and understand English.
Must pass a pre-employment background check and drug test.

Nice To Haves

At least one year of experience in a social services department in a skilled nursing care facility or other related medical facility is preferred.

Responsibilities

Assist in planning, developing, organizing, implementing, evaluating, and directing the facility’s social service programs.
Participate in discharge planning and in developing and implementing social care plans and resident programs.
Interview residents and families as necessary, in a private setting.
Complete required forms and reports and submit them to the Administrator as required.
Involve residents and families in planning social service programs when possible.
Assist in arranging transportation to other facilities when necessary.
Refer residents and families to appropriate social service agencies when the facility does not provide needed services.
Provide residents and families with information about Medicare, Medicaid, and other available financial assistance programs.
Provide consultation to staff and community agencies to help address residents’ needs and problems through social service programs.
Perform other duties as necessary and appropriate or as directed by the Administrator.
